Volunteer Background Check Release Form - Just as we are committed to developing the spiritual walk of our children, GCBA is committed to protecting the welfare of our children.  Because of this we are requiring background checks on all summer participants 18 years and older.  Here you will find a pdf copy of a Background Check permission form.  Please have all adults coming with you on this mission trip fill out the form and send to Dionne Williams via any of the methods listed (fax 228-832-0995, email to dionne@gulfcoastba.com, snail mail to 11201 Old Hwy 49, Gulfport MS 39503). All information will be kept strictly confidential and only viewed by those necessary to conduct the background search and appropriate leader(s) and/or staff.  If your church already performs checks on volunteer workers, please feel free to submit copies of those checks.  GCBA wants to be a good example to her churches of being responsible in all areas where the safety and welfare of our children is concerned.

Medical Release/Activity Consent Forms GCBA provides this as a service to you.  We DO NOT require you to have consent forms.  It is best for you to prepare in case of a medical emergency or plan to have your group participate in outside activities.  A consent form must be signed by a parent or legal guardian, not by the group leader.  If possible, have the signatures notarized.  Consent forms do not change your liability in case of an accident but does help you establish prior knowledge of activities by those responsible.
